A pictorial bookplate on sepia tinted paper with black ink for Leo John Madsen. There is an illustration of a desk in front of bookshelves. On the table is a lamp, books, and papers. The illustration is framed in an oval. On the top of the oval are ribbon banners that say "One of the books of" with a skull in the middle. On the bottom of the oval are two medieval scientific tools. Above the skull on the left is Phi Delta Chi of the Harrison School of Pharmacy's crest with their motto in Latin "alterum alterius auxilio eget" which translates to "Each Needs the Help of the Other." On the left is a two headed eagle with a banner that says in Latin "Deus Meumque Jus" which is a masonic motto that can be translated as "God and My Moral Rightness." In between the crest and eagle is a caduceus, the medical symbol of two snakes coiled around a staff. On top of these illustrations is an hourglass that has angel-like wings.
