Abstract

This evidence-based quality improvement initiative evaluated the applicability and system-level impact of a confidential, QR-enabled mobile HIV risk self-assessment implemented within a military primary care setting. Despite mandated HIV screening and universal access to pre-exposure prophylaxis (PrEP), engagement in prevention services remains inconsistent due to structural and institutional barriers. A technology-enabled intervention was deployed to improve access to risk assessment and facilitate patient-initiated engagement with care. Aggregate data were analyzed over a six-month period following implementation to assess QR code utilization, HIV testing rates, and PrEP prescribing trends. QR engagement and immediate changes in HIV testing or PrEP prescribing were not observed; however, the intervention demonstrated operational feasibility and highlighted structural factors influencing prevention behaviors within the Military Health System. These findings support the need for sustained implementation strategies and system-level approaches to enhance HIV prevention engagement and readiness outcomes among active-duty personnel.
